To schedule or change an appointment, please call the office where you wish to be seen.

Please bring the following to your first appointment:

• Patient's insurance card
• A valid photo ID
• Physician referral forms if required by insurance
• A list of current prescriptions and/or over-the-counter medications, including dose and frequency
• Pertinent information about your medical and surgical history
• Any recent MRIs and CT scans or appropriate records
• Completed patient forms
