To make an appointment, please call the location where the patient will be seen:

Please have the following information available:

• Patient's full name, date of birth and social security number
• Patient's address and phone number(s)
• Type of insurance
• Reason for the visit
• Referring provider's full name and telephone number

If you prefer, you may schedule an appointment by completing and faxing this referral form to us. We will contact the patient the same day and notify your office of the appointment date and time.

For your patients' convenience, you may also print our new patient information and forms, and directions to our offices.

Please provide us with the patient's most recent office note and any labs or testing that is pertinent to his/her visit with us. These may be faxed to 859.255.0740 (Lexington) or 859.936.0306 (Danville).
